Please note that you should untick the download of the toolbar and Photoshop unless you really need those.  (I can recommend the Photoshop software if you do use your computer for digital photographs as it is a good piece of software for removing red eye and cropping and resizing photographs).  Please note that if you still use dial up then the download will take around 45 minutes.  (Anybody who has a problem with this format please email me on the link at the bottom of this page and I can send the equivalent Word document).

These forms are produced so that club secretaries can print off the forms and then complete manually, thus saving time if these forms were requested from the appropriate league official.  (PLEASE NOTE THAT THOUGH SOME OF THESE FORMS USED TO HAVE TO BE SENT IN ON VARIOUS DIFFERENT COLOURED PAPER IT IS NOW ACCEPTABLE TO USE THESE DOWNLOADED FORMS ON PLAIN WHITE PAPER AS LONG AS THEY HAVE THIS YEARS DATE ON THE HEADER I.E. 2007/08

These files should be downloaded to your own PC and used as a template each time they are required.
